This form may be used by faculty and staff only for on campus events in academic spaces or public buildings. Event coordinators should review the AV Services Policy before completion.

NOTE: Faculty and staff may also request to borrow digital video and still cameras, laptops, and data projectors through the IT equipment loan program.
