This form may be used by faculty, staff and student organization representatives* for on campus events in academic spaces or public buildings. Please review the A-V Services policy first!

*Student organization representatives should use this form only for events occurring during business hours in academic or public spaces (no residences); otherwise, please make an A-V equipment loan request.

  *Setup time indicates when you prefer for the equipment be set up in the room. Actual setup time, however, is often determined by factors beyond our control (e.g. other events in that space, previously scheduled setups, etc.).

^Faculty and staff may also request to borrow digital video and still cameras, laptops and projectors through the I.T. loan pool.
#Student organization representatives may also request to borrow data/video projectors and portable screens from the HelpDesk.
